MAIN FUNCTION: The Housing Case Manager is a member of the Mino Kaanjigoowiin team, a Housing First intensive case management team working with Aboriginal clients who experience chronic mental health and addiction issues. This program is a part of the Na-Me-Res (Native Men’s Residence) range of programs working with clients experiencing homelessness.

DUTIES AND RESPONSIBILITIES INCLUDE:

  • Provide case management services which adhere to the principles of Housing First, harm reduction and strength-based recovery. Follow clients from homelessness to housing, providing appropriate client driven supports regardless of their current housing situation
  • Complete thorough intake, assessments, and develop healing plans with clients using a strengths-based, recovery-oriented approach, and monitor client progress toward their goals
  • Ensure the accurate maintenance and completion of case management records and paperwork, in a timely manner
  • Collaborate with members of the interdisciplinary team to provide exceptional services and outcomes
  • Provide clients with appropriate community referrals to address physical, mental, emotional and spiritual needs. Support each client in their own community and attend appointments with clients when required
  • Liaise, advocate and ensure timely communication on behalf of clients with community agencies, including health providers and landlords
  • Ensure that program delivery is consistent with Aboriginal culture and traditions
  • Prevent and/or resolve developing crises with clients in a timely manner to avoid unnecessary use of emergency rooms, hospitals, the justice system and shelters
  • Maintain client confidentiality and boundaries
  • Travel using TTC as required
  • Attend all team meetings and training
  • Other related duties and identified and required by the Manager or leadership team

REQUIREMENTS:

  • Post-Secondary education in Social Work or related fields or equivalent experience
  • Ability to act as a key contact for Mino Kaanjigoonwiin team regarding housing
  • Possess a firm grasp of addiction theory and harm reduction services
  • Registered member of respective Ontario College, in good standing
  • Strong capacity to manage crises and problem solve
  • Excellent interpersonal skills
  • Knowledge of Housing First Principles
  • Knowledge of Aboriginal social and health related issues including mental health and addiction issues with 3-5 years’ experience
  • The successful candidate to provide a current CPIC check as a condition of employment
  • Ability to communicate in American Sign Language is an asset
