Salary:                    $25 to $28 per hour, depending on qualifications and experience

POSITION SUMMARY

Working within the Homemaking Framework, the Indigenous Care Coordinator assesses client needs, determines client eligibility for services, develops plans of service with clients, and coordinates home making services to older adults (55+) or adults with age related health conditions and/or disabilities and/or who require respite. These services will be delivered in a culturally safe and appropriate manner that ensures the best health outcomes for clients.  Non-medical homemaking services will be offered in the areas of Barrie, Orillia and Muskoka.

JOB REQUIREMENTS

All employees of BANAC are required to submit a clean Criminal Record Check with Vulnerable Sector upon hire and confirm compliance every year thereafter.  All employees are to comply with the Occupational Health and Safety Act (OHSA), its regulations and all occupational health and safety policies and procedures.  Due to the requirements of this position, a Valid Ontario Driver’s License and use of insured vehicle is required.  Valid CPR and AED Certificates are required.  Due to the nature of this position working some evening and flexible hours maybe required.

QUALIFICATIONS

  • Diploma in Native Community and Social Development, or Social Services Worker, or health related field with a minimum of 2 years’ experience.
  • Preference shall be given to persons of First Nations or Indigenous heritage who meet the educational requirements, or who have alternate, equivalent education and experience in the opinion of the Executive Director.
  • Valid Ontario Driver’s License and use of insured vehicle is required.
  • Ability to speak an Indigenous Language, an asset.

PREFERRED QUALIFICATIONS

  • Experience with completing Health Assessments using the RAI-CHA tool, an asset.
  • Experience working with seniors or adults with age related health conditions and or disabilities.
  • Experience working with Indigenous communities.
  • Experience and knowledge in Indigenous cultural approaches and traditions.
  • Indigenous Cultural Safety Training an asset.
  • Experience working with mental health and addictions.

SKILLS

  • Strong communication and interpersonal skills.
  • Effective planning, organization and evaluation skills.
  • Ability to manage multiple clients, assess their needs and goals, develop plans of care and coordinate services.
  • A solid understanding of and sensitivity to the experiences of First Nations and Indigenous peoples.
  • Knowledge of community resources including Indigenous specific resources.
  • Ability to compile statistics and write narrative reports.
  • Computer proficiency, with knowledge of assessment software and Microsoft Office.

PHYSICAL DEMANDS AND WORKING CONDITIONS

  • Regular travel to meetings and communities.
  • Low to moderate exposure to noise in the work environment.
  • Periodic exposure to smoke may occur from the burning of sacred medicines: Tobacco, sweet grass, sage or cedar.

EMPLOYMENT CONDITIONS

Specific employment conditions are specified in an employment contract with the Executive Director and include the following:

  • Valid C.P.R. and A.E.D. Certificates required.
  • Valid Ontario Driver’s license and access to a reliable insured vehicle is required.
  • Clean criminal record check with vulnerable sector screening is required.
  • Due to the nature of this position working some evening and flexible hours is required.
