Youth Navigator- Niagara Region

(St. Catharines, Fort Erie, Niagara Falls)

9 Month Contract Position (40 hours per week)

Vision Statement

The urban Aboriginal youth we serve will have increased confidence, self-esteem and leadership abilities. They will develop skills and a strong attachment to the workforce that will make them more employable and provide them with greater stability. NPAAMB will be a leader in supporting our youth as they transform into leaders contributing to their communities.

The Youth Navigator will primarily be responsible for developing and providing wrap around case management services to urban Indigenous youth by developing a plan of care that is community oriented, family centered, strengths based, and reflects the cultural values and practices of each youth. The Youth Navigator will provide support to urban Indigenous youth who are experiencing difficulties navigating or are unaware of the resources in their community.

QUALIFICATIONS

  • Post-secondary education in one of the following disciplines: Social Work, Education, Sociology, Child and Youth Studies, and a minimum of five years of work experience delivering services, preferably to Indigenous youth; or
  • In lieu of education, a minimum of 10 years’ experience delivering services, may be considered
  • Knowledge of the wrap around service delivery model
  • Knowledge of principles of social counselling
  • Knowledge of Indigenous cultures, traditions, and their histories
  • Knowledge of social issues impacting urban Indigenous youth today
  • Working knowledge of office computer systems including: Windows 10 OS and MAC iOS.

Applications include: Office 2016 (includes Word, Excel, Outlook, PowerPoint) for MAC and Windows, and Office 365 with Sharepoint Online and One Drive. Also a working knowledge of business use of smart phones and mobile devices is required.

MAIN RESPONSIBILITIES

Counselling and Case Management:

  • Prepare and oversee facilitation of a plan of care that addresses the social issues impacting urban Indigenous youth
  • Ensure the wraparound process is completed with high conformity to the model and track progress toward goals
  • Coordinate and facilitate client wrap around meetings
  • Encourage and help urban Indigenous youth, who have difficulty navigating community programs
  • and services.
  • Develop and maintain client files inclusive of all required documents and maintain accurate case notes
  • Research community resources and make referrals to these service organizations
  • Ensures that all required confidentiality requirements are met.

Administration

  • Participate in mandatory wrap around training
  • Arrange payments for child care, transportation and hospitality expenses related to wrap around sessions
  • Attend bi-weekly meetings with the Program Manager to communicate the progress of activities and provide monthly narrative reports of progress
  • Assist in all aspects of the project deliverables as directed by Supervisor
  • Provide back up to the Youth Service Officer as requested/directed

The successful candidate must have a valid driver’s license, ability to work overtime periodically, and ability to travel to fulfill the duties of the position.
