JOB SUMMARY: The Diabetes Nurse reports to and works under the direction and supervision of the Team Supervisor, Community Health and Wellness, Six Nations of the Grand River Elected Council. Receives clinical day-to-day supervision from the Clinical Lead.

PURPOSE & SCOPE OF THE POSITION:

The role of the Diabetes Nurse is to provide supportive and collaborative chronic disease management services in the context of diabetes. These services can be one-on-one or in a group setting, provided in the environment that best addresses the needs of the individuals seeking services.

BASIC QUALIFICATIONS:

• Graduate of a Bachelor of Science in Nursing

OR

• Graduate of a Diploma in Nursing
  • Must be registered with the College of Nurses of Ontario
• Certified Diabetes Educator preferred
