JOB SUMMARY:

The Health Transformation Community Engagement Coordinator reports to and receives direction from the Director of Health Services.

The Health Transformation Community Engagement Coordinator is responsible for facilitating and coordinating an inclusive community engagement strategy for the Six Nations Health Services (SNHS) Health Transformation Project with a focus on building and maintaining relationships with community members and service providers. The Health Transformation Community Engagement Coordinator provides leadership within the framework of the Haudenosaunee Wellness Model, and in alignment with the Six Nations strategic vision in the context of the broader Canadian health care system. The Health Transformation Community Engagement Coordinator is a member of the Six Nations Health Services (SNHS) – Health Leadership Team (HLT) and is responsible for planning, organization and coordination of the designated portfolio within the department and overseeing the administration and operation of their assigned area;

The Health Transformation Community Engagement Coordinator is accountable for liaising between stakeholders through a community engagement strategy in support of the strategic direction of the Health Transformation Project. This role is responsible for developing relationships through targeted outreach and increasing community awareness of SNHS various programs, services and initiatives to facilitate service improvement dialogue;

The Health Transformation Community Engagement Coordinator will also accept responsibility for fostering an environment that has cultural values and principles of practice as foundation within the policies and procedures established by Six Nations of the Grand River.

BASIC QUALIFICATIONS:

  • University graduate or Post-graduate Degree in relevant field
  • Knowledge and experience working in Hodi:noshoni/Rotinonhsion:n communities
  • Knowledge of Indigenous issues and available service providers within Six Nations and neighbouring communities
  • Enthusiastic and positive attitude about changing systems and community perspectives
  • Demonstrated ability to solve problems and manage conflict
  • Work experience with effective engagement strategies and tactics
  • Ability to build and manage positive relationships with committees and networks (both internal and external), community partnerships, professional organizations and other stakeholders
  • Ability to manage large projects and facilitate complex interactions that involve stakeholders from across the system
  • Strong facilitation and positive collaboration skills; ability to network with community resources, build relationships and create community partnerships
  • Comprehensive knowledge of standards of care, professional standards, best practice, evidence-informed decision-making, legislative requirements, risk management and quality improvement
