Mental Health Outreach Worker
Health & Family Services / Wellness Building
The purpose of this position is to provide one-to-one counselling, group programming to community members and to provide prevention, assessment, referral, and follow-up services to individuals and families affected by mental health and substance use issues.
The duties and responsibilities of this position are categorized into four main functions:
1. Client Counselling
- Provide mental health one on one counselling and group programming regarding cause, symptoms and prevention strategies
- Facilitate the delivery of assigned programs utilizing appropriate community and external resources
- Service coordination (linking service recipients with services and supports)
- Provide outreach counselling in the community
- Provide assertive outreach and rapid response to clients who may be experiencing psychosis, completing initial intake screening and assessments
- In collaboration with client, family and the interprofessional team, develop a recovery- oriented treatment plan
- Provide clinical case management for individuals experiencing symptoms of first episode psychosis, and their families
- Continually monitor and evaluate the experience of meaningful recovery, improved quality of life, and achievement of goals, and make changes to the treatment plan for clients as necessary
- Provide crisis intervention and promote stabilization
- Provide best practice mental health interventions for psychosis, including individual, group and family counselling approaches, and social/vocational/educational rehabilitation support
- Develop and facilitate psychosocial and recreational group programming for clients
- Advocate at the person’s request for needed services and supports
- Integrate psychosocial rehabilitation principles, promoting skills training and social integration
- Coordination of Community Treatment Orders (CTO)
2. Documentation and Case Management
- Ensure client files are complete and accurate including progress notes
- Complete intakes and discharge of clients
- Provide crisis intervention and behavior management techniques
- Ensure confidentiality of all clients (past and present) is maintained
- Participate in case meetings as applicable
- Maintain monthly client progress summary to Manager of Mental Health and Addictions
- Complete assessments as required
3. Administration
- Collect information for statistical purposes specific to clients and program
- Review reports to ensure information is complete and accurate
- Address critical incidents as per internal reporting mechanisms
- Participate in staff development activities as required
- Maintain accurate and complete verbal and written exchange of information to represent all activities while on duty (logs and reports)
- Participate actively in the case management process
- Prepare accurate and professional client case notes
4. Provide Community Education, Support and Activities
- Coordinate and facilitate educational workshops on mental health related topics
- Attend and sit on community and organizational committees as requested
QUALIFICATIONS: Basic Requirements: (APPLICANTS MUST SHOW NECESSARY PROOF WITH APPLICATION OR WILL BE AUTOMATICALLY SCREENED OUT)
EDUCATION:
- A Master’s degree is preferred with minimum 2 years directly related work experience providing clinical case management
- A University degree in Social Work or related field with minimum 2 years directly related work experience providing clinical case management
- A College diploma in Social Services or related field with minimum 5 years related work experience, education and training may be considered
OTHER RELATED REQUIREMENTS:
- Thorough knowledge and understanding of First Nation history and culture
- Knowledge of Curve Lake First Nation resources and area agency resources for referral purposes
- Excellent interpersonal and counselling skills to work with a diverse client group
- Possess good time management, written and verbal communication skills, exercise good judgement
- Ability to instruct clients in relevant programs
- Experience and/or knowledge of report writing and file maintenance
- Analytical ability and effective problem-solving skills in assessing client needs
- Crisis intervention abilities (ingenuity and resourcefulness in developing alternative strategies to meet individualized programming)
- Behavioral management skills
- Demonstrated ability to model social skills
- Demonstrated ability to advocate for clients
- Possess excellent computer skills including Microsoft Word, Excel, Access and PowerPoint
The incumbent must also demonstrate the following personal attributes:

TERMS OF EMPLOYMENT:
This is a Full-Time Permanent position for 39.5 hours a week beginning immediately. Annual salary range for this position is $64,522 – $68,824.
