JOB SUMMARY: The Personal Support Worker reports to and works under the direction of the Case Manager of the Case Management Unit assigned to their work area within the Home and Community Care Program under the Six Nations Health Services Department.

To assist the Health Services Department and other Personal Support Workers in the performance of statutory, operational, and advisory services duties related to the Six Nations Home and Community Care Program; within the policies and procedures established by the Six Nations of the Grand River Elected Council for the Six Nations Home and Community Care Program.

To assist the Case Manager of the Case Management Unit assigned to their work area by providing home care services for individuals and families of Six Nations of the Grand River Elected Council Territory during periods of illness, incapacitation, convalescence, or family disruption.

BASIC QUALIFICATIONS:

• Personal Support Worker Certificate.
• Must have a minimum of Grade 10.
• Must have a vehicle and valid Class “G” driver’s license.
• Current certificate in First Aid and CPR.
